Under the general supervision of Community Wellness Center leadership, the Community Wellness Specialist serves as IEHP’s front facing ambassador, providing exceptional customer service and ensuring a positive first point of contact for all visitors. This role supports daily center operations, database management, program implementation, policy and procedure workflows. The Community Wellness Specialist tracks key metrics and collaborates with internal and external partners to ensure mobile clinics—such as immunization events, mammogram screenings, and well child visits—meet regulatory requirements and contribute to quality goals. The position also engages directly with participants through presentations and one on one interactions, helping advance IEHP’s commitment to proactive, member centered care, integrated wellness, and community empowerment.

Key Responsibilities

  1. Provide welcoming front desk support by serving as the center’s concierge, assisting members and visitors through walk ins, inbound/outbound phone calls, and initial touchpoints to support all Center offerings. Register participants, explain required documents such as waivers and rules of conduct, and route members to appropriate services using approved referral pathways. Maintain clean, organized, and inviting center spaces, promote programs and resources, and support community engagement activities and events to ensure a positive overall member experience.
  2. Conduct daily quality reviews and maintain accurate productivity reports across systems to support the Quality Assurance Program. Develop program presentations aligned with organizational strategies and provide high quality front-line support by guiding guests through available services, assisting with intake and scheduling needs, reinforcing approved health education materials, and communicating member needs to clinical staff to ensure coordinated, seamless care.
  3. Support the development and delivery of training materials, schedules, and curriculum while maintaining effective communication with team members and other departments. Lead data collection efforts for center and participant surveys, analyze results to recommend program enhancements, and create program cards using approved platforms to support referral and outreach initiatives. Ensures contractor compliance and the provisions of the contract are fulfilled.
  4. Coordinate and implement the participant rewards program, assist members in accessing rewards, and identify opportunities for improvement by gathering feedback and monitoring program effectiveness. Contribute to strategic initiatives that enhance member experience, streamline workflows, and strengthen Vibrant Health’s community impact by providing insight from front-line interactions, help implement new processes or program, to ensure alignment with organizational goals. Create program cards for referral intakes using Access Database, Connect IE, and/or other software platforms to support outreach initiatives.
  5. Perform any other duties as required to ensure Health Plan operations and department business needs are successful.

Qualifications

Education & Requirements 

  • Minimum of two (2) years of experience in any of the following:
    • Customer service in a managed care setting
    • Managed Care/IPA/Clinic Setting
    • Community based organizations
    • Community engagement and outreach
  • High School Diploma or GED required
  • Associate’s degree from an accredited institution preferred

 

Key Qualifications

  • Must have a valid California Driver’s license and valid automobile insurance
  • Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points
  • Knowledge of Medi-Cal/managed care environment preferred
  • Understanding of and sensitive to multi-cultural communities
  • Proficiency in Microsoft Office Suite and member tracking systems (e.g., MediTrac, Medhok)
  • Knowledgeable in effective presentation practices for internal and external stakeholders
  • Interpersonal and communication skills, including but not limited to, appropriate communication, conflict resolution, excellent teamwork, speaking confidently and clearly to large audiences, and motivational interviewing, with a commitment to equity and inclusion
  • Professional skills, including but not limited to, balancing multiple projects, constant learning, observing ethical and legal rules, and identifying situations calling for mandatory reporting
  • Ability to build effective working relationships
  • Ability to exercise adequate time management and strong ability to multitask
  • Proficient writing ability
  • Must be able to lift up to 50 lbs.
  • Ability to work flexible hours, including occasional evenings and weekends

Pay Range

USD $25.90 - USD $33.02 /Hr.
