Reporting to the Manager, Integrated Transitional Care Medication Management, the Integrated Transitional Care Clinical Pharmacist is responsible for the daily operation of IEHP clinical programs and activities. The Integrated Transitional Care Clinical Pharmacist provides comprehensive care to Members through IEHP Clinical Programs including Post-Discharge Medication Reconciliation and other departmental initiatives.

Key Responsibilities

  1. Complete medication reviews for post-discharge medication reconciliation and other medication management programs that includes reviewing chart notes, electronic medical records, and/or relevant clinical information within turnaround times and quality standards meeting regulatory requirements as defined by Department of Health Care Services, Center for Medicare & Medicaid Services, and National Committee for Quality Assurance. Provide additional clinical oversight for activities related to the post-discharge medication reconciliation and department needs.
  2. Review and respond to inter- and intra-departmental tasks sent via MedHoK, Meditrac, Care Director, or any other systems required and coordinate the care of the member as necessary with the other departments such as Member Services, Health Education, Behavioral Health & Care Management, Compliance, etc. Communicate clearly and effectively with Providers and/or Members on issues related to the medication reconciliation, or other respective policies and procedures. Demonstrate excellent customer service.
  3. Perform peer review auditing and quality assurance checks to ensure clinical decision making and clinical recommendations meets the regulatory and company standards.
  4. Participate in interdisciplinary teams (Physician, Case Manager, Nurse, Social Worker, etc.) to discuss the ongoing needs of IEHP Members, which include, but not limited to medication regimen review and consultations. Attend case management discussions and/or rounds to review medication therapies and provide medication assessments.
  5. Provide clinical education, guidance, and support to maintain productive working relationships with Team Members, other Providers, and management team and ensure adherence to standard work.
  6. Actively participate in internal process and quality improvement process enhancements, LEAN projects, and managing for daily improvement (MDI). Ensure all departmental goals (including, but not limited to turnaround, production expectations, and quality metrics) are met.
  7. Participate in various department projects to meet Performance and Quality measures (including, but not limited to standards of Healthcare Effectiveness Data and Information Set, Consumer Assessment of Healthcare Providers and Systems, and National Committee of Quality Assurance). Ensure all company’s goals related to the IEHP Strategic Plan are met.
  8. Perform any other duties as required to ensure Health Plan operations and department business needs are successful.

Qualifications

Education & Requirements 

  • Two (2) or more years of individual or combined experience in pharmacy, hospital, or drug benefit related quality assessment, utilization management and/or provider liaison duties OR a minimum of one (1) year Managed Care residency OR Ambulatory Care residency
  • Doctor of Pharmacy (Pharm. D.) from an accredited institution required
  • Possession of an active, unrestricted, and unencumbered Registered Pharmacist license issued by the California State Board of Pharmacy

 

Key Qualifications

  • Strong clinical baseline knowledge of current medical treatment protocols and clinical practice guidelines
  • Working knowledge of regulatory standards and timelines for Medi-Cal, Medicare, and Covered California lines of business
  • Bilingual (English/IEHP Target Language) preferred
  • Proficiency with health plan electronic medical records systems (i.e. MHK, Meditrac)
  • Excellent written and verbal communication skills
  • Strong drug information research skills
  • Excellent communication and interpersonal skills
  • Proven ability to:
    • Participate in interdisciplinary teams
    • Exhibit behavior as referenced in the IEHP Handbook (i.e., attendance, dress code, electronic device policy, personal behavior)
    • Be detail-oriented
    • Communicate and work effectively with a variety of stakeholders and maintain positive working relationships with internal and external contacts at all levels
  • Schedule may adjust based on business need which may include evenings, holidays, and weekends

Work Model Location

Telecommute (All IEHP positions approved for telecommute or hybrid work locations may periodically be required to report to IEHP’s main campus for mandatory in-person meetings or for other business needs as determined by IEHP leadership)

Pay Range

USD $154,128.00 - USD $204,214.40 /Yr.
