The Community Health Worker - Social & Community Service (SCS) will be responsible assessing the health risks and identifying gaps pertaining to Social Determinants of Health (SDoH) which create barriers and contribute to the wellbeing of a Member's care needs. Under the general supervision of the Social & Community Services leadership, the Community Health Worker - SCS will be responsible for supporting Members in improving their health through outreach and engagement activities either via phone and/or in the field. 

In collaboration with the Member, the Community Health Worker - SCS will take ownership of Social Determinants of Health (SDoH) goals, will be responsible for facilitating health, risk and safety screenings, health education activities, and special events to promote health for Members and community as well as help Members access the best types of care for their needs. 

 

This is a Bilingual (English/IEHP Threshold Language) position. The Team Member in this role must take a language proficiency assessment and must receive a passing score.

Key Responsibilities

  1. Assess Members' Social Determinants of Health (SDOH) through the lens of person-in-environment, and provides support, further evaluation, or coaching as needed to support Members in prioritizing their needs and goals.
  2. Assist Member socio-economic issues that affect their overall health and develop health/social management plans and goals.
  3. Review and assess health screenings based on utilization of approved screening tools and collection of health history and refer individuals with abnormal results to the appropriate facilities for further evaluation before they escalate.
  4. Provide appropriate healthcare services and resources for Members and demonstrate a deep understanding of the cultural and linguistic diversity within assigned area.
  5. Assist Members in effectively navigating the healthcare system by providing education on the health plan, coverage, and benefits; guiding the selection of appropriate care providers; coordinating medical and behavioral health appointments and referrals; clarifying medical terminology and offer ongoing support to ensure optimal health outcomes. 
  6. Foster cultural sensitivity and be an advocate on behalf of IEHP Members in the home, the community, and in provider organizations.
  7. Deliver educational sessions on health promotion, preventive care, chronic disease management, healthy lifestyle choices, and available community resources to empower individuals in making informed healthcare decisions in ways that the community can easily understand, culturally and linguistically, and provide information on IEHP Member benefits and services.
  8. Educate Members on selection of immediate care options to include information on using an Emergency Room versus other immediate care options, such as urgent care, nurse advice line, etc. Help Members in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
  9. Coach Members in effective management of their chronic health conditions and self-care in understanding care plans and instructions so to educate clinical teams and Providers on Member specific culture, norms, beliefs and preferences to help bridge language and communication barriers, ensuring that healthcare information is accurately understood and conveyed.
  10. Motivate Members to be active and engaged participates in their health and overall wellbeing, aid in medication management, emphasize preventative care, and offer guidance on lifestyle choices.
  11. Model commitment to continuous quality improvement by engaging in quality improvement initiatives and preventative health projects, tailored to address local MCAS & HEDIS gaps, and test new practices for improving the outcomes of the Community Health Department.
  12. Responsible to document all client encounters and contacts made on behalf of clients. Complete and submit monthly reports; maintain comprehensive electronic client files, which include client notes, assessments and other medical documents acquired on behalf of the client.
  13. Participate in care coordination and case management as well as ad hoc case conferences when needed. Make referrals and provide follow-up.
  14. Assist clinical teams and Providers to understand the culture, norms, beliefs and preferences of the Members and their community by representing the voice of the community and delivering these messages in a way that fits the culture of Members.
  15. Attend community events and health fairs to complete social needs screenings, resource coordination, and information sharing about IEHP and community programs and services.
  16. Participate in all appropriate staff meetings or other activities as needed.

Qualifications

Education & Requirements 

  • Two (2) years of experience as a CHW, Promotora, or Health Navigator, or two (2) years of experience working in community outreach, case coordination, delivering presentations/training to varied audiences, customer service, or within a medical office
  • Experience working in a multi-cultural community and understanding of those
  • Experience working in a Behavioral Health or Substance Use Disorder program preferred
  • High school diploma or GED required
  • Successful completion of an approved Community Health Worker (CHW) program or will complete within six (6) months of hire
  • This is a bilingual position. Identified bilingual team members in this role must take a language proficiency assessment and must receive a passing score.

 

Key Qualifications

  • Must have a valid California Driver's license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points
  • Must have knowledge in the following areas:
    • Surrounding social determinants of health and familiar with community resources within the SB/RIV counties
    • Health care services
    • Thorough understanding of the Community Health Worker role and skillsets needed to succeed in the workforce
    • The community the CHW will be working in, especially community resources, and their reputation in the community
    • Awareness and sensitivity to mental health conditions and addictive disorders
    • Awareness of the impact of unmitigated bias and judgement on health; commitment to addressing both
    • Understanding of, and a commitment to, high preforming team practices

  • Excellent communication and interpersonal skills, including but not limited to the ability to connect with an audience, the ability to speak using clear and plain language, active listening, and strong writing ability

  • Build and maintain a positive working relationship with Providers
  • Computer skills, including typing, data entry, managing email, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint)

  • Strong planning, organization, detailed oriented time management skills and critical thinking skills
  • Time Management skills, ensuring documentation is accurate, timely, useful and in compliance with regulatory requirements and accreditation standards
  • Maintain appropriate written and oral communication on a timely basis, completing documentation within 24 hours of activity, and returning non-urgent calls within 48 hours

  • This role requires extensive traveling in assigned communities. This position will be required to travel routinely to client homes, community locations, various agencies, and other outreach destinations. The Community Health Worker - SCS must be willing to have a flexible work schedule, often requiring evening and weekend hours. Standard business hours are 8am-5pm but will vary by need. High amount of field work with 75% driving travel required in assigned territories in Riverside and San Bernadino Counties. Must be willing and able to travel to Members' homes in addition to working in an office environment as needed. Must be available for work on nights and weekends
  • CHWs hired in Rural positions would telecommute and/or work from a collocated partner site as designated by leader
  • Position is eligible for telecommuting/remote work location upon completing the necessary steps and receiving HR approval. All IEHP positions approved for telecommute or hybrid work locations may periodically be required to report to IEHP’s main campus for mandatory in-person meetings or for other business needs as determined by IEHP leadership.

Pay Range

USD $25.90 - USD $33.02 /Hr.
