Under the general direction of Health Equity Leadership, the Health Equity Programs Liaison is responsible for supporting short and long term operational and strategic Health Equity initiatives. This role will coordinate and administer IEHP health equity programs and initiatives aimed at addressing the needs of the members and the community with a focus on addressing and eliminating health disparities. As a Health Equity Program Liaison, this role will develop relationships/partnerships with community leaders and community-based organizations (CBO), serving as the Health Equity Department’s liaison with the community. This position engages in outreach to CBOs, spreading information about IEHP’s community efforts, advocacy, and engagement opportunities. This role is a valuable member of the department to improve health equity and reduce health disparities. The Health Equity Program Liaison actively supports the Consumer Advisory Committee (CAC) and the Health Equity strategy. They will routinely communicate organizational policies, procedures, and processes, anticipate and respond to health equity needs and conduct ongoing CBO/Member/Provider and public entity trainings.  The Health Equity Program Liaison is a subject matter expert on programs that promote diversity, equity, and inclusion across systems of care.

1. Facilitate and increase community interaction and partnerships with external organizations to meet requirements for Health Equity Disparities strategy plan and NCQA Health Equity Plus.
2. Develop and maintain positive working relationships with all internal and external stakeholders to ensure optimum member care and satisfaction. This includes: IEHP Providers, County employees, community-based organizations, minority communities and marginalized populations: transgender, non-binary, and SPD members. Support Community Information Network (CIN) efforts by identifying and onboarding CBOs onto the CIN.
3. Follow up with CIN participating CBOs and improve bidirectional communication between participating entities by addressing barriers and mitigation strategies.
4. Plan, coordinate, and conduct internal and external trainings including, but not limited to: Health Disparities, Disability Sensitivity, Independent Living Philosophy, and specific subject matter trainings as requested by IEHP Team Members to improve communication, support and accessibility for minority groups including seniors and persons with disabilities.
   1. Plan and coordinate diversity awareness events and activities for IEHP Team Members, Providers, and CBOs to train and inform about topics relating to diversity, equity, inclusion and access, such as cultural humility, sex and gender minorities, people with disabilities, seniors, individuals with limited-English proficiency and populations experiencing health disparities.
5. Manage assigned Health Equity Programs and/or actively participate on other team projects supported by Health Equity.
6. Administer member health equity programs including, but not limited to outreach calls, social media campaigns, and member heath education initiatives to address prioritized health and social health disparities.
7. Collaborate with internal departments in supporting the Community Advisory Committee. 
8. Provide field intelligence for Health Equity and Disparities strategy work plan.
9. Write and disseminate progress reports and presentations based on assigned Health Equity projects.
10. Provide guidance on IEHP collaterals through appropriate channels to ensure alignment with Health Equity Strategy.
11. Represent IEHP and participate at Community Resource Center events, community events, community meetings, provider events and health fairs, as needed.
12. Contribute to the development of programmatic goals, objectives, policies, and procedures; assists in program development and tracking. Identify programmatic challenges and assists in designing or revising program operations and procedures to improve efficiencies. Conduct formal and informal training on IEHP benefits, features, and managed care concepts to State contracted Medi-Cal enrollees, County employees, IEHP Provider staffs, and community program staff.
13. Serve as a liaison to major Department of Social Services Offices, Area Agencies on Aging, Social Security Offices, Hospitals, and other organizations where potential synergies exist.
14. Plan and coordinate meetings and events for the Inland Empire Disabilities Collaborative (IEDC).
    1. Plan and coordinate monthly IEDC meetings including, but not limited to, programming, marketing, networking, and logistics.
    2. Plan and coordinate the IEDC Expo and Conference including, but not limited to, logistics, fundraising, attendance, budgeting, marketing, entertainment, audiovisual, and exhibitor development.
15. Any other duties as required ensuring Health Plan operations are successful.

Education & Requirements 

• Two (2) years social, community, communication, health plan or market/sales services related work experience required
• Bachelor’s degree in Human Development, Public Health, Sociology, Social Work, Psychology, Human Services or any related field from an accredited institution required
• Must be able to satisfactorily pass IEHP’s Medi-Cal Marketing test within three (3) months of employment

Key Qualifications

• Must have a valid California Driver's license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points-person meetings or for other business needs as determined by IEHP leadership
• Knowledge and familiarity with resources available in both Riverside and San Bernardino Counties such as housing assistance, food programs, crisis counseling services, etc. is required
• Knowledge of cultural humility concepts, Americans with Disabilities Act principles and equity and diversity best practices in healthcare impacting racial, sex and gender minorities and populations with limited English proficiency
• Understands health care services for seniors and persons with disabilities (SPD)
• Has knowledge of Americans with Disabilities Act
• Understanding and sensitivity to multi-cultural communities
• Bilingual (Spanish/English/Chinese/Vietnamese) preferred
• Verbal and written, word processing and spreadsheet capabilities
• Data input and basic analysis
• Excellent interpersonal/communication skills, public speaking, and presentations skills with communities of diverse education, socio-economic, cultural, and professional backgrounds
• Organizational ability
• Telephone courtesy and high degree of patience
• Strong problem-solving capability
• Must be willing to work evenings and weekends
• Must be willing to travel
• Health Equity Program Liaison will have on-going, face-to-face and group connections, conversations, and presentations with IEHP members, general community, community/faith-based agencies, other partnering agencies and leaders
